I’ve tonnes of model releases, and for all sorts of reasons, I prefer a good old fashioned, hand signed form. Of course, they’re all filed and piled, but it’s still a mess. So here’s my solution:

The layout was done by copying my original text into a master page in InDesign. I then added the logo and Model Release as a header in the master file. Then I simply added 199 pages based on the master page. Finally I added left and right numbers for alternating pages. Lulu took the exported PDF directly too.. Sweet!
